Intervention description

The Fusion delivery model is based on a team approach with staff members specialising in different aspects of the support package offered to young people. Following referral Intervention Workers provide holistic and informal support to individuals throughout the programme supported by the wider team. 

Approach to specifying intervention

Black box approach. Projects were granted flexibility in designing and implementing their own intervention approaches, informed by the pattern of existing provision and the specific challenges presented by local housing and employment markets. Key commonalities included using a "housing-led" approach, a "similar participant journey" and support from key workers.

Further information

Referral routes: Each of the three local authorities covered by the project (Metropolitan Borough of Kirklees, Metropolitan Borough of Calderdale and City of Wakefield Council), had their own referral gateway arrangements, with Assessment and Outreach workers engaging with young people and assessing their needs and aspirations using the Outcomes Star. Referrals were also taken from a range of other organisations in each locality. These referrals were then passed on to the appropriate gateway for verification.
