Intervention

Single Point of Access - the Oldham Integrated End of Life Care Hub – to improve the access to end of life care across the borough. The service would provide a single contact point for health professionals, patients and families, 111 and the VCSE sector to call, and provide 24 hour support to ensure that patients receive appropriate care in the community.
